Your actions can make a positive and tangible difference – one step at a time.

All people with diabetes should work to achieve the knowledge, skills and abilities necessary for diabetes care so that they can implement and sustain the behaviors needed for on-going self-management both upon diagnosis and as needed thereafter.

Diabetes self-management education (DSME) is a critical element of care for all people with diabetes and is necessary in order to improve your outcomes. This process incorporates the needs, goals, and life experiences of the person with diabetes and is guided by evidence-based standards.

This education is patient-centered, respectful, and responsive to your individual needs and values, and it will empower you in making medical decisions for yourself.

This participation is recommended at diagnosis of diabetes, annually, when complication factors arise and when transitions in care occur, such as hospitalizations.

Getting your education for diabetes self-management is highly recommended by the Institute of Medicine National Committee on Quality Assurance, the American Diabetes Association and by the Endocrine Society.

The overall objectives of our education program in offering you individual classes are to support informed decision-making, self-care behaviors, problem-solving and active collaboration with the health care team; all of which should improve your clinical outcomes, health status and quality of life.
